We are seeking a compassionate and dedicated Patient Care Coordinator to be a part of the multidisciplinary psychiatric treatment team. The PCC is directly involved with psychiatric inpatients, completing psychosocial assessments, treatment plan development, contact with family and collateral supports and arranging all aspects of the aftercare disposition. The PCC additionally provides therapeutic support to patients in both individual and group therapy formats.

Qualifications

  • Master's Degree in Psych, Counseling, Social Work on related field
